Nondiscrimination & Language Assistance

Sanevi Health Solutions complies with applicable federal civil rights laws — including Section 1557 of the Affordable Care Act, Title VI of the Civil Rights Act, Section 504 of the Rehabilitation Act, the Age Discrimination Act, and the Americans with Disabilities Act — as well as the Illinois Human Rights Act (775 ILCS 5). We do not exclude people or treat them differently because of race, color, national origin, age, disability, sex (including pregnancy, sexual orientation, and gender identity), religion, marital status, military status, or source of payment.

1. Services we provide free of charge

  • For people with disabilities: qualified sign language interpreters, written information in other formats such as large print or accessible electronic documents, and reasonable modifications to our policies and visit format.
  • For people whose primary language is not English: qualified interpreters and information written in other languages. We do not ask patients to bring their own interpreter and do not rely on minors to interpret.

2. Gender-affirming and inclusive care

We use your correct name and pronouns regardless of what appears on your insurance card, and we record them in your chart. Teens and adults are treated with the same respect. Tell us at intake how you want to be addressed and how we should contact you.

3. How to file a grievance

If you believe we failed to provide these services or discriminated against you, you may file a grievance with our Civil Rights Coordinator: 1515 N Harlem Ave, Suite 101, Oak Park, IL 60302, (708) 948-7039, docs@sanevihs.com. We will help you complete the form if you need assistance, and we will not retaliate against you.

4. Filing with the government

You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, electronically at ocrportal.hhs.gov, by mail at 200 Independence Avenue SW, Room 509F, HHH Building, Washington, DC 20201, or by phone at 1-800-368-1019 (TDD 1-800-537-7697). Illinois complaints may be filed with the Illinois Department of Human Rights.
