The content on this site, including focus-area pages and self-check questionnaires, is general educational information. It is not medical advice, a diagnosis, or a treatment recommendation, and using this site does not create a provider–patient relationship. A relationship begins only after we accept you as a patient, verify identity and consent, and complete an evaluation with a licensed clinician.

This site is not monitored around the clock and cannot be used for emergencies. If you are in danger or thinking about harming yourself or someone else, call or text 988, call 911, or go to the nearest emergency department.

Our screeners are adapted from publicly available instruments (for example GAD-7, PHQ-9, ASRS v1.1, ISI, EPDS, PSS-4). They are screening aids only. Scores can be high or low for reasons unrelated to a clinical condition, and only a licensed clinician can diagnose. Results shown on this site are not part of a medical record until a clinician reviews them with you.

• Submit accurate information and only about yourself or a person you are authorized to act for.
• Do not attempt to access data belonging to others, probe our systems, or interfere with the site.
• Do not upload malware or use the referral portal for anything other than legitimate clinical referrals.
• Referring professionals must confirm they have the authority or consent needed to share the records they upload.

Our clinicians are licensed in Illinois. Care — including telehealth — is available only to patients physically located in Illinois at the time of the visit, unless we tell you otherwise in writing.

These terms are governed by the laws of the State of Illinois, without regard to conflict-of-law rules. Disputes will be brought in the state or federal courts located in Cook County, Illinois.
